At this clinic, a medical provider will complete and sign the Medical Report that is required to submit the Ohio State Dental Board initial dental license application. This clinic is only for DDS or DHY students who plan to obtain their dental/hygiene license in the state of Ohio after graduating.
We will have all necessary paperwork for the completion of the Medical Report and proof of hepatitis B immunity. There is a $20 paperwork completion fee associated with the appointment that is due at the time of service and is NOT billable to insurance.
